Blue Cross Blue Shield ties $942 million in extra billing to AI-assisted coding

Blue Cross Blue Shield Association estimated that AI-assisted hospital coding contributed to nearly $942 million in extra costs for its health plans between 2023 and 2025, with about 70%, or $653 million, tied to extra diagnoses that did not change a patient's care. BCBSA's Luke Chalker said "multiple factors contribute to coding intensity," but added that the findings suggest AI-enabled coding tools are playing a role.
NoteHospital coding is one example of a wider pattern: when AI speeds up a process built on an existing financial incentive, it can scale whatever bias is already in the system as easily as it scales efficiency, which is a caution for AI-driven credit and claims models too.
